Thyroid-stimulating hormone (TSH) is often the first blood test; free T4 measures available thyroid hormone. Our evaluation may include free T3, another thyroid-hormone measurement, and thyroid antibodies when clinically indicated. Not everyone needs the same tests. Reverse T3 is not routinely useful for assessing an underactive thyroid. Symptoms and medicines also guide interpretation.
